Transitional Primary Care
The post discharge line, built for the weeks after a hospital or skilled nursing stay in the metro.
Transitional Primary CareDenver service area
Clinician appointments happen over video, and Denver sits inside a twenty-five-mile Medical Assistant region, so a Medical Assistant may be able to come out for vitals or to get the technology working.
Care is delivered by telehealth, with Medical Assistant support within twenty-five miles of Denver.

After the discharge
The Denver metro is the densest hospital and rehabilitation discharge market in Colorado, so a large share of the state's discharges happen right here. The days that follow one of those stays are when the medication list changes, when a new symptom shows up, and when nobody is quite sure who owns the next decision.
Transitional Primary Care exists for that gap. A clinician reads the discharge summary, walks the medication list line by line, asks how the first days at home have actually gone, and puts the next step in writing. Because the visit is by video, it can be scheduled around the patient's week rather than around a waiting room.
Working alongside
Most Denver patients arrive with people already involved. A discharging facility, a home health agency scheduling nurse visits, a specialist, sometimes a primary care office that has known them for twenty years. Nobody is asked to switch.
With the patient's authorization, the clinician can pass along what came out of the video visit, so the same medication change is not discovered twice by two different people. Where a long-term-care or rehabilitation facility participates with Home Doc, in person rounding may also form part of the arrangement. That is answered facility by facility rather than across the metro.
The Denver radius
The Medical Assistant region is drawn twenty-five miles out from Denver. Aurora, Lakewood, Littleton, Northfield and Central Park all sit inside it, along with much of the surrounding metro. Within that ring a Medical Assistant may be available to record blood pressure, pulse, weight and temperature in person, or simply to sit with a patient who cannot get the video call to connect.
Availability is a scheduling question rather than a guarantee. Intake checks the address, the requested service and the calendar before anyone is promised a visit.
A Medical Assistant is not a clinician. The physician, physician assistant or nurse practitioner joins by video, and no Home Doc clinician travels to a patient's address.

Video, always
Every physician, physician assistant and nurse practitioner appointment is delivered by telehealth.
In person rounding happens only at select participating long-term-care and rehabilitation facilities.
What to have ready
The Denver intakes that move fastest are the ones where the paperwork is already in hand and the caregiver is on the line.
01
Whatever the hospital or rehabilitation unit sent home, including the medication list and any results still pending.
02
The home health agency, the specialist, the primary care office, and the family member who should be on the call.
03
Home Doc works with plans from Medicare, Medicaid, Aetna, Cigna and UnitedHealthcare. Naming an insurer is not the same as covering every plan or service, so the exact product is verified first. Detail sits on the insurance page.

Palliative CareDenver is one of two areas where a Medical Assistant can come to you; Colorado Springs is the other. Everywhere else in Colorado, visits happen by video. When you are ready, become a patient.
No. The Northfield Blvd address is where Home Doc runs intake and administration. Appointments with a physician, physician assistant or nurse practitioner take place by video, wherever the patient happens to be.
Timing turns on eligibility, coverage verification and clinician availability rather than on travel, which is usually what stretches the wait out. Intake can describe the next opening once the paperwork has been reviewed.
With the patient's authorization, yes. Coordinating with an agency, a facility case manager or an existing specialist is a routine part of the transitional line.
Only if that particular facility is one of the select participating long-term-care and rehabilitation sites where in person rounding has been arranged. Otherwise she is seen by video, and facility staff can help her get connected.
Both fall within twenty-five miles of Denver, as do Littleton, Northfield and Central Park. Intake still confirms the specific address, because the edge of the region runs through neighborhoods rather than politely around them.
The card itself and the exact product name. Home Doc lists plans from Medicare, Medicaid, Aetna, Cigna and UnitedHealthcare, and an insurer appearing there does not mean each of its plans or every service is covered.
Next step in Denver
Intake will ask what care you are after, where you are, what the coverage looks like and which records exist. One call is usually enough to know whether this fits.
