Send a referral
Referrals can be submitted by email or fax. Use either to start a referral or to ask a question about fit, coverage, or timing.
Include the face sheet, the last office note, and the medication list with every referral.
When to refer
Anyone involved in the patient's care can start a referral, including facility staff, home-health teams, primary care practices, specialists, patients, and families. Referring early gives intake time to review coverage, contact information, and records before the first visit is needed.
What happens next
Sending a referral does not automatically enroll a patient or book a visit. Intake reviews what you send, contacts the patient or authorized representative, and confirms service fit, consent, and coverage before anything is scheduled.
Keep protected health information out of general website contact forms and other unsecured messages. Send it by intake email, phone, or fax using your organization's approved secure process.
Intake reviews available information and confirms the best follow-up method.
The team contacts the patient or authorized representative to discuss interest, service fit, and consent.
Insurance, discharge information, medication lists, and relevant records are reviewed as available.
An appointment is coordinated when eligibility, service fit, and provider availability are confirmed.
Once care begins, Home Doc may communicate with the referring organization and other clinicians when permitted and clinically appropriate. The scope and frequency of updates depend on the service, patient authorization, and the roles of the organizations involved.
What to include

Hospitals, rehabilitation teams, facilities, home-health organizations, primary care practices, specialists, community professionals, patients, and families may contact intake.
Referrals may be considered for Transitional Primary Care, Psychiatric Care, and Palliative Care.
The team prioritizes prompt outreach, but response and appointment timing depend on referral completeness, ability to reach the patient, eligibility, insurance, and provider availability.
Home Doc is designed to complement existing clinicians and support continuity. It may discuss a standalone primary-care role when a patient does not have access to an established provider.
A clearer next step
Tell the team what care you need. Intake can discuss fit, coverage, records, and next steps.
